    Понятия, имеющее особое значение в представлениях Винникотта о развитии в раннем возрасте. Истинная Самость представляет собой "врожденный потенциал", составляющий "суть" ребенка. Непрерывность развития истинной Самости и ее становление облегчается достаточно хорошей матерью, обеспечивающей ребенка здоровой средой и соответствующим образом отвечающей на его сенсомоторные "запросы" и потребности Самости. Кроме того, достаточно хорошая мать создает условия для адекватного удовлетворения Оно ребенка, когда его влечения организованы в виде функционирующей системы. Истинная Самость развивает свой индивидуальный язык с помощью материнской поддержки и опеки, поддерживающей непрерывность и постоянство существования ребенка и помогающей ему разработать индивидуально-экспрессивные качества на основе стержневых характеристик Самости и индивидуального чувства реальности. Винникотт рассматривал истинную Самость в связи со спонтанной репрезентацией Оно. Поэтому внешние проявления Самости, согласно Винникотту, в феноменологическом смысле так же "неуловимы" и эфемерны, как и репрезентанты инстинктов.
    Ложная Самость, подобно Я, представляет собой стабильную, обратимую и постоянно функционирующую структуру. Винникотт полагал, что некоторые индивиды страдают своеобразными расстройствами ложной Самости — так, в частности, можно рассматривать шизоидный характер, — но он также считал, что такое разделение Самости на истинную и ложную является совершенно нормальным. Таким образом, истинную и ложную Самость следует, согласно Винникотту, соотносить не с морально-этическими качествами личности, а с особенностями восприятия себя и других, поддерживающих спонтанную экспрессию (истинную Самость) либо "реактивные" аспекты жизни (ложную Самость).
    Проявления ложной Самости могут указывать на отсутствие или "сокрытие" истинной Самости, что обычно наблюдается у шизоидных индивидов. Если мать не способна удовлетворять сенсомоторное "Оно" ребенка и ориентируется не на то, что есть в реальности, а на свои чувства, то у ребенка формируется ложная Самость. Апперцепция берет верх над перцепцией.
    Каждый индивид обладает социальной Самостью, соответствующим определенной части структуры ложной Самости и формирующимся из нее. На другом конце этого спектра находится индивид, оперирующий в основном ложной Самостью, соответствующей тому, что Хелен Дойч (1942) описала в качестве "как будто личности". С ложной Самостью часто связана интеллектуализация.
