    Перверсия, приписываемая только лицам мужского пола и характеризующаяся воображаемым либо действительным переодеванием в женскую одежду с целью достижения психологического комфорта или возбуждения. Процесс переодевания нередко является необходимым условием коитуса или мастурбации. Как правило, трансвеститы используют несколько предметов, а порой располагают целым гардеробом соответствующих вещей, включая парики. Переодевание может проявляться в виде одиночных актов, помогающих преодолеть чувство вины, либо достигать широкого размаха, слагаясь в определенную субкультуру у Я-синтонных личностей. Переодевание для разных индивидов имеет разное значение; чаще, однако, функции трансвестизма сводятся к развитию специфического компромиссного образования, выражающего идентификацию с образом женщины, наказание за инцестуозные эдиповы желания и фантазию о фаллической женщине. Мастурбация, особенно в кульминационный момент оргазма и семяизвержения, позволяет "одержать победу" над идентификацией с женщиной и над кастрацией.
    В отличие от гомосексуализма и транссексуализма переодевание при трансвестизме носит выраженный фетишистский характер; при этом выбор гетеросексуального объекта и принадлежность к мужскому полу остаются относительно адекватными. Длительные катамнестические наблюдения показывают, что большинство мальчиков, склонных к переодеванию в женскую одежду (эта тенденция проявляется уже на втором году жизни), чаще становятся гомосексуалистами. Стремление к переодеванию проявляется у трансвеститов в латентный период или в раннем подростковом возрасте. Именно в этом возрасте переодевание становится основным элементом эротизма, хотя то, в какой степени эта активность поддерживается Я или имеет доступ к его исполнительным аппаратам, варьирует.
    В некоторых случаях разграничение трансвестизма и транссексуализма оказывается затруднительным, поскольку почти все трансвеститы, не будучи транссексуалами, фантазируют на тему превращения в женщину. По мере созревания сексуальные аспекты переодевания становятся все менее значимыми, уступая место создающей психологический комфорт стойкой идентификации с женщиной. У определенного контингента трансвеститов зрелого возраста, в частности у тех, у кого снижена самооценка, могут наблюдаться состояния острого расстройства защитных функций. Такие мужчины могут приобретать черты транссексуалов и стремятся к изменению половой принадлежности. Они склонны к суициду и самооскоплению, и их состояние следует рассматривать как требующее неотложной медицинской помощи.
