Макушка головы. Off the top of one's head — без предварительной подготовки, спонтанно. Выражение вошло в употребление в конце 1930-х гг. и пришло из мира радио, а позже из телевизионных программ. Режиссёр передачи обычно похлопывал себя по макушке, показывая тем, кто находился в студии, что программа опережает расписание, имеет лишнее эфирное время и что им нужно быстро найти дополнительный материал, чтобы заполнить это время.I don't know exactly how old he is.
