106 аффективные расстройства
Группа нарушений, в которых ведущим симптомом является аномальное усиление аффективных реакций. К этой группе относятся клинические состояния, характеризующиеся депрессией, возбуждением либо их чередованием. За исключением расстройств, где преобладающим аффектом является возбуждение (мания, гипомания, маниакальная фаза маниакально-депрессивного психоза), понятия аффективных и депрессивных расстройств синонимичны. Синдромы с преобладанием аффекта тревоги рассматриваются отдельно, однако тревога присуща всем психическим заболеваниям и в разной степени проявляется и при упомянутых нарушениях.В разделе "Основные аффективные расстройства" "Диагностического и статистического руководства" Американской психиатрической ассоциации (DSM-III) приводятся такие психотические состояния, как мания, депрессия и биполярные расстройства (чередование маниакальных и депрессивных эпизодов). В раздел "Другие специфические аффективные расстройства" включены циклотимия, при которой маниакальные и депрессивные фазы не достигают психотического уровня, а также дистимия (или депрессивный невроз), когда относительно устойчивая депрессия является недостаточно длительной и выраженной, чтобы соответствовать критериям основного депрессивного эпизода. Кроме того, в DSM-III описываются атипичные биполярные расстройства, атипичные депрессии и регуляторные нарушения с преобладанием подавленного настроения. По некоторым данным, биполярные расстройства обусловлены генетически и являются следствием особой биохимической аномалии. Некоторые люди генетически предрасположены к депрессии, но психологические факторы являются, пожалуй, важнейшим элементом в ее возникновении и упрочении.В психоаналитической литературе отдается предпочтение традиционной, сложившейся за многие годы терминологии, основанной на типичных для этих расстройств психодинамических констелляциях, которые описаны в данной книге в рубриках "депрессия", "возбуждение" и "мания". Мания, депрессия, маниакально-депрессивный синдром и инволюционная депрессия являются психоаналитическими терминами для обозначения состояний, соответствующих аффективным расстройствам по классификации DSM-III, хотя некоторые аффективные симптомы могут проявляться в связи с шизофреническими, пограничными и невротическими нарушениями. Психоаналитическая категория депрессивный невроз соответствует циклотимическим, дистимическим и реактивным депрессивным расстройствам по DSM-III. (Последнее расстройство представляет собой острую, но ограниченную по времени реакцию на ситуацию, представляющую определенную угрозу для индивида.)
