Человек считается членом АА, если сам считает себя таковым. У него нет никаких обязательств. Членство основывается на признании "двенадцати шагов", с помощью которых АА старается помочь своим членам излечиться. Они сводятся к следующим положениям: "Мы признаем свое бессилие и знаем, что сами не сможем излечиться. Мы обсуждаем свое положение с другими. Мы стараемся загладить свою вину перед людьми, которым мы причинили зло. Мы обращаемся с молитвой о помощи к Высшей Силе, как каждый из нас ее понимает. Мы стараемся помогать другим алкоголикам, не щадя сил и не думая о вознаграждении". В США подход АА лежит в основе всех авторитетных курсов лечения от алкоголизма. Средства АА поступают в основном от издания брошюр, книг, буклетов и т.н. "Большой книги" (главного труда, публикуемого регулярно с 1939 года под названием "Анонимные алкоголики"). В организации более 2 млн. членов (2004). Штаб-квартира в г. Нью-Йорке.
